Support at Home program

The Support at Home program is a great option to assist you to maintain your independence and remain living safely at home for longer by receiving tailored services on an ongoing basis.

It suits people with more complex care needs or who require a greater range of care and support services to be coordinated and managed with the assistance of a dedicated Care Advisor.

The Support at Home program is available to Australians aged 65 and over (or 50 and over for First Nations and Torres Strait Islander Peoples) deemed eligible by a federal aged care assessment.

Once your Support at Home program is approved, you will be allocated a package based on the level of care you require.

The Support at Home program offers a wide range of services so you can continue to live well at home and remain healthy, active and socially connected in your community.

You will have access to an approved list of services and be supported to choose or change the mix of services you can access within your budget and that best meet your needs. 

Under Support at Home, there are eight ongoing service classification levels and corresponding government-subsidised funding levels (budgets) available. You will be allocated a support at home classification based on your aged care assessment and eligibility.

Your Uniting AgeWell Care Advisor will work with you to navigate the Support at Home program, optimise your budget and adapt services as your needs change.  Together, we will create a comprehensive support system to help you thrive at home.

Your individual Support Plan allows access to three service categories – Clinical Care, Independence and Everyday Living. 

Support at Home services may include: 

Clinical Care

• Nursing services
• Allied health (physiotherapy, occupational therapy, podiatry, etc.)
• Nutrition support
• Clinical care management

Independence

• Personal care
• Social support
• Respite care
• Transport
• Assistive technology

Everyday Living

• Domestic assistance
• Home maintenance
• Meal preparation and delivery
• Garden maintenance
