Restorative Care Pathway

This 12-week program is a short-term and more intensive block of therapy and support than your usual programs. The program is designed in partnership with you to reach your individual goals and you can apply for the program to be extended for up to 16 weeks if needed.

This program is designed to have an emphasis on Allied Health and clinical support. Some examples of typical supports include:

• Exercise physiology or physiotherapy services
• A home safety assessment and advice
• Nursing support
• Group or telehealth exercise classes
• Dietician
• Podiatry
• Speech pathology

If you have a Care Advisor through Uniting AgeWell already, they will be able to assist you to apply for this pathway

If you do not already have services with Uniting AgeWell you can call our friendly team on 1300 783 435 for assistance.

Alternatively, you could take the steps below: 

1. Register with My Aged Care and undergo an Assessment
2. They will refer you to us for the Restorative Care Pathway
3. We will contact you to complete your onboarding with us and commence your program.

The Restorative Care Pathway is a new program, starting from 1 November 2025, and replaces what was previously called the Short-Term Restorative Care program (STRC). 

If you are part of the STRC program your care will be transitioned to the Restorative Care Pathway.
