As you age, it's important you remain connected to the things that matter to you and continue to feel valued members of your community. Uniting AgeWell's social connections programs help you age well at home.
Our Social Connections optionsUniting AgeWell's Day Centres and Planned Activity Groups support people to meet and mix with others through social and leisure activities and outings.
Our ‘Well for Life’ programs provide an opportunity for exercise, strength training and health education.
We also offer centre-based programs for people living with dementia, and targeted small group social and leisure activities.
Uniting AgeWell receives funding from the Department of Health and the Commonwealth Home Support Program to provide planned activity groups for eligible seniors living in the community.
Programs are flexible and responsive to the individual’s needs and provide an environment that encourages and supports the maintenance of daily living skills and opportunities for social interaction.
Uniting AgeWell has developed a dementia checklist in conjunction with Barwon South West Department of Health and Human Services.
This practical tool is specifically designed to ensure people living with dementia can experience social support programs that reduce the effects of dementia and increase opportunities for meaningful participation.
It can also be used by any social connections program.
In addition, we offer a vibrant activity group for people in the Geelong region with younger onset dementia. The Hello Monday group supports people to keep active and connected in their community.
It also gives carers a break and an opportunity to share experiences with others in a welcoming environment.

Operating out of Aldersgate Village in Newnham, Launceston, each Saturday from 9am to 4pm, Launceston AgeWell Centre offers social support and day respite for community clients over the age of 65.
