End-of-Life Pathway

Extra care and support when you need it most.

Overview

For many people, home is where we feel most at peace — surrounded by familiar faces, cherished memories and the comfort of our own space.

As the end of life nears, whether through illness or the natural process of ageing, the wish to remain at home with dignity and support is deeply important.

Uniting AgeWell can provide enhanced care and peace of mind for older Australians wanting to remain in the comfort and familiarity of their own home as they approach the end of life.

Our holistic support encompasses physical, psychosocial and pastoral care according to the wishes, needs, culture and beliefs of each person and their family.
