Assistive Technology and Home Modifications scheme

Through the AT-HM scheme Uniting AgeWell will work with you to provide assistive technology designed to make your daily tasks easier.

For more complex equipment, Uniting AgeWell’s allied health professionals will carry out a health assessment to prescribe the items that best meet your needs. 

Assistive technology may include: 

• Mobility aids: walking sticks, frames, wheelchairs, mobility scooters 
• Toileting supports: bedpans, commodes, incontinence alarms 
• Bathing equipment: shower chairs, bath boards 
• Cognitive aids: memory support tools, medication reminders 
• Personal safety items: anti-slip footwear, alarms, body protection.

Through the AT-HM scheme Uniting AgeWell will work with you and a team of health professionals to provide a health assessment to prescribe the right home modifications to meet your needs.

Home modifications may include: 

• Basic modifications: grab rails, handrails, ramps 
• Complex modifications: bathroom redesigns, stair lifts, doorway widening for wheelchair access. 

The AT-HM scheme is one of three short-term funding pathways that may be accessed through the government-subsidised Support at Home program.

The AT-HM scheme is funded separately to any ongoing services you may receive through the Support at Home program. 

This means you will not need to save money from the budget from your Support at Home program to apply for assistive technology or home modifications.

AT-HM scheme funding will cover the assistive technology or home modifications you need, a health professional assessment if required and any extra support from the Uniting AgeWell team to ensure you are able to use the items safely and effectively. 

You can access up to $15,000 for home modifications and up to $15,000 (or more with evidence) for assistive technology over 12 months. It will not accrue over time. 

You may be eligible to receive one or both types of funding, with the amount received based on your assessed needs. 

This will cover access to an approved list of products, equipment (such as a wheelchair) and home modifications (such as a shower rail) to meet your assessed needs.

In some circumstances, such as if you have a progressive condition, you may be eligible for more time to use your AT-HM funding. 

If your needs change and you require additional assistive technology after your funding has expired you can be re-assessed for further funding. Our team can assist you to apply for this additional funding.
