Home Care Packages

A Home Care Package is a great option to assist you to maintain your independence and remain living safely at home for longer by receiving tailored services on an ongoing basis.

It suits people with more complex care needs or who require a greater range of care and support services to be coordinated and managed with the assistance of a dedicated Care Advisor.

Home Care Packages are available to Australians aged 65 and over (or 50 and over for First Nations and Torres Strait Islander Peoples) deemed eligible by a federal aged care assessment. Once your Home Care Package is approved, you will be allocated a package based on the level of care you require.

A Home Care Package offers a range of tailored care and support services.

At your initial home care visit, a Uniting AgeWell Care Advisor will clearly explain how your Home Care Package works and what to expect.

Your Care Advisor will work alongside you to develop a care plan that supports you to live life on your own terms and help you navigate the care and funding landscape.

There are currently four levels of Home Care Package support available from basic to high-level care.  

While Home Care Packages are Government-funded, an income assessment may be necessary to determine the level of subsidy you are entitled to. You can also opt to privately fund your services.

There are many ways Uniting AgeWell can support you to live independently and safely at home. 

Depending on the level of Home Care Package you are allocated and your individual needs and preferences, you can choose to access a range of services. These may include:

• Personal care, including help with showering and dressing
• Domestic support, including help with household chores
• Shopping for groceries and meal preparation
• Home or garden maintenance or home modifications to maintain safety and independence
• Transport for shopping, medical appointments or social outings
• Provision of assistive technology including remote monitoring systems, mobility aids and personal safety alarms
• In-home nursing, such as medication management and wound care
• Other services like social support, allied health and more.
